I am one of 5 Advanced Practice Providers that serve as the sole on call provider when working in the 4 bed Emergency Department of this Critical Access Hospital located in historic Deadwood, SD. Our ED sees a wide variety of medical and traumatic emergencies across the age spectrum, from local residents to visitors and tourists enjoying the beautiful Black Hills of South Dakota. We care for a high volume of traumatic injuries, related to the winter and summer recreational opportunities this area affords.

Employed as ALS/Critical Care provider for municipal third service EMS provider to the home of the Sturgis Motorcycle Rally. BLS, ALS, Critical Care services provided during 911 and interfacility transports, ALS intercepts for surrounding communities. Provision of patient assessment, interventions, documentation, initiation of billing documentation. Provide initial and continuing education to EMS providers and students at the BLS, ALS, Critical Care levels. Population of the community varies fro 6,700 people to 500,000+ during the rally.

Member of the Sturgis Volunteer Fire Department functioning as a firefighter conducting structural and wildland fire suppression, technical rescue including high angle, vehicle, and ice/coldwater rescue. Emergency vehicle operation including driving and pump operation of engines, tenders, command vehicles. Promoted to Fire Lieutenant for a period of time, functioning as company officer during these same operations, rotated as on call duty officer for the department, provided education to members of the department.

I was the sole Provider on duty of this 7 bed ED, within a Critical Access Hospital located in in east-central South Dakota. As such I directed and provided care to a culturally diverse community comprised of hispanic residents and a refugee population of immigrants from Myanmar known as the Karen, as well as the other members of the community.

Hired as Flight RN for this rotor and fixed wing EMS provider in western SD, provided scene and interfacility BLS, ALS and critical care to injured and ill patients of all patient populations. Conducted assessments, provided interventions, and documented care, participated in billing process, quality assurance. Promoted to Medical Education Coordinator, conducted new hire orientation and continuing education to RN and Paramedic medical crew members, as well as outreach education to local EMS and hospital providers, initially locally, then regionally and nationally for Air Methods Corporation, the parent company.

Volunteer ALS provider with community based, not for profit EMS service located in the Northern Black Hills of SD. Provided direct patient care, documentation, stocking, and assisted with education of EMS providers.

Member of VFD, including providing wildland and structural fire suppression, emergency vehicle operation including engine, tender, ambulance, and quick response vehicles. Appointed as Rescue Captain, oversaw EMS and technical rescue functions and budget of the department. Developed and oversaw implementation of transition from basic life support to advanced life support licensure. Developed and implemented volunteer incentive program including college education reimbursement. Developed continuing education program. Envisioned, developed, and implemeted ALS staffed medic response vehicle, staffed internally by volunteers.

Employed initially as a flight paramedic for this rotor wing EMS provider during scene and interfacility transportation of acutely ill or injured patients. Span of care included OB, newborn, pediatrics, adolecents, young through older adults, geriatrics. Completed direct patient care including assessments, interventions, transportation, documentation, quality assurance functions, in collaboration with a RN partner. Maintained competence and functioned in the ED and ICU, hospital wide code team when not flying. performed ground critical care transports, in collaboration with local ground EMS when transport by air was not available or advisable. Transitioned to RN role following graduation with BSN and licensure as RN.

Developed and conducted EMS, fire suppression, and life safety educational programs to employees of contracted companies, primarily servicing the off-shore oilfield industry. Classes included water survival, H2S awareness, onboard firefighting, EMS classes.

Provision of BLS and ALS patient care during 911 responses for the designated EMS provider to the parish. Conducted assessments, performed interventions, transported patients as both care provider and vehicle operator, completed patient documentation, particpated in first level QI processes.

Provided BLS and ALS assessments, patient care, and documentation to patients and facilities requesting transportation, primarily based in Baton Rouge service area. Popualtion served was primarily interfacility, though back up 911 response to assist primary providers also occured frequently.

Functioned as EMT-Paramedic for this hospital based ALS first response agency. Responded to calls for ALS intercept from surrounding BLS EMS agencies, conducted assessments, performed interventions, completed documentation, functioned within the hospital emergency department when not on calls

Assisted with the presentation of didactic and practical coursework in compliance with AHA and CHoP guidelines and direction, completed evaluation of students, completed required paperwork.

Functioned as a EMT-Paramedic for this ALS EMS agency that was the primary 911 provider for this community and the surrounding area. Performed patient assessments, conducted interventions, completed documentation.

Functioned as adjunct instructor for initial and continuing education classes offered by the CEM including EMT-B, Paramedic, ACLS, BLS. Participated as adjunct instructor in the pilot class for the DOT 1994 EMT Curriculum revision project.

Garrison MP duty, gate security, patrol duties, patrol supervisor, desk sergant. Provided security and law enforcement to residents and vistors to facilities assigned to the Military District of Washington, including Ft. Myer, Ft. McNair, Cameron Station, Davidson US Army Airfield (Ft. Belvoir), as well as special events at Arlington National Cemetery such as wreath layings at the Tomb of the Unknowns. Protection detail assignments included President Reagan, Prince Charles & Lady Diana, et.al.

When working, I am the sole provider on call for ED patients, in addition to directing care for the acutely hospitalized patients of this 16 bed Critical Access Hospital located in northeastern WY. Responsibilities include assessment, evaluation, diagnosis and treatment, as well as documentation of care of patients presenting for care at this facility located on Interstate 90.
