Directing the management of care and direct patient care of post open heart surgical and peripheral vascular patients based on physician orders and needs of the patient. Performed these duties on adults. Also have experience with ventricular assist devices for patients awaiting heart transplants. Took care of some post heart transplant patients. Also had direct patient care of other chronic health conditions, such as sickle cell anemia and dialysis.

Performs patient care as needed in facilities such as hospitals and long term care. Currently performing teaching for the INRatio 2 INR meter to patients on Coumadin.

Performed all duties within the Medicaid population of members following state and federal law. Performs care management for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum and ensuring member access to services appropriate to their health needs. Provide health education to member's with complex and chronic health issues focusing on Diabetes to promote better self-care management. Assisted with expediting prior authorizations by obtaining medical records from such entities as physicians and home health agencies. Use of Milliman Guidelines for case management duties and assistance with prior authorization process.
