Provided therapy services to patients in SNF setting for NHC Healthcare including, geriatric orthopedic patients, traumatic brain injuries, CVA's, amputees, spinal cord injuries, and generalized weakness. Also, provided skilled therapy services to long term care patients and residents, attended care conferences with families when needed, and documentation of progress of patients in Casamba software program.

Responsibilities include, daily treatments of patients, implementation of POC and carryover, documentation of progress towards goals, communication with supervising Physical Therapist regarding patients or any modification to POC or goals, discussion with other treating clinicians including Occupational and Speech Therapy on patients, educating support staff and caregivers, and attending rehab team meetings. All of these duties have been requirements of four facilities within middle TN area including Lebanon, Gallatin, White House, and Gainsboro,TN.

Provide therapy services to patients in Skilled Nursing Facilities in Rutherford and Robertson counties as a PRN therapist while providing skilled services to a variety of patients. Additional duties are providing flexibility with scheduling of daily patients and employees going on vacation, to fufill any other needs the company may have such as pay system requirements, to maintain high efficiency and productivity, and to provide daily documentation and weekly progress reports in Rehab Optima 4 program software.
