Public Health Nurse (PHN) – Teen Hope Program I work with the Teen Hope Program at MVNA. As a Public Health Nurse I provide teen mothers who are still in high school under the age of 20 case management, care coordination, quality education and support related to pregnant women and parenting skills. I assist mothers and their partners with their newly delivered infants, by providing resources for gaining the skills to be a successful parent and raising a well adjusted child. I work to assure that the mothers attend school and the child is in daycare. My clients are culturally diverse and reside in Minneapolis and the greater Hennepin County.
