I am currently a north set senior nurse for a complex and additional needs care provider. We assist CCGs, CHC's, health boards, clients and their families to create bespoke packages of care to enable clients be cared for in the home environment. We provide complex, specialist health and social care in the community through comprehensive care planning and management of support plans and delivery of services to promote the independence and well-being of each individual at all times, enabling the client to lead a fulfilling and inclusive lifestyle.
