Diff rates (2019) and 2 years of zero harm for CLABSIs by leveraging quality clinical database to build new patient care pathways—low rates were sustained during COVID-19 • Decreased readmission rates for discharged patients by 90% by developing and implementing Transition of Care clinic • Received rare award by Integrated Healthcare Association for top performance in Ambulatory Quality, Cost Measures and Patient Experience in 2020 and 2021 • Awarded Gold Plus honor from American Heart Association for innovations in stroke care following implementation of new stroke telemedicine program—recognized by U.S. News and World Report for “High Performing” in 6 specialty conditions including stroke care
