Plan, promote and implement health education programs for the WIC program, including determining eligibility, evaluate lab values, nutritional requirements, and prescribe food packages. Perform nutritional assessments of participants and families by evaluating anthropometric measurements, lab values, nutritional, medical and social histories. Implement nutrition education classes and food demonstrations weekly. Develop care plans, counsel and monitor the progress of intervention in a health unit. Provide 1:1 coaching to customers around their behavior changes to improve health status, reduce health risks and improve quality of life by using motivational interviewing and behavior change techniques. Document interactions and interventions, collect and assess biometric data, and conduct group education on multiple health topics. Support customers working in conjunction with a team of other health care providers.

Develop individualized programs for patients interested in maintaining wellness, improving nutrition intake and health status. Help patients wishing to receive continuation of care and health advancement after diagnosis, as well as supplement, support, and personalize medical care. Help patients understand their health conditions, while integrating new goals into the patient’s total lifestyle and promote healthy behaviors. Provide patients with health information, nutrition education, food demonstrations, and motivation to achieve their goals. Referred patients to other health care providers and developed relationships to promote the clinic services within the community. Coordinate client services, participate in development and evaluation of treatment plans, manage client case records and documentation, provide selected counseling services, screen clients referrals for program appropriateness, participate in staff meetings, and participate in educational activities such as workshops and seminars. Conduct outreach into the Hispanic community to develop opportunities for educational activities, as well as to identify program partner organizations.
