• Completes clinical nutritional assessments on all patients as required, and within timeframes established by Federal regulations. • Evaluates the nutritional status of patients by evaluating nutritional histories and intakes, establishes long-term nutritional goals and objectives for individual patients, records findings, and attends patient care planning conferences. • Teaches patients, and/or families, as well as Dietary staff, therapeutic diets when requested. • Visits patients routinely to assess and monitor nutritional status. • Completes progress notes on each high-risk patient according to standards set by the company. • Manage dietary services for assigned living centers. • Collaborate with interdisciplinary care team to meet nutritional needs of each resident. • Conduct individualized nutrition assessments based on laboratory data and medical history. • Review records and assess the nutritional needs for at-risk residents. • Plan medical nutrition therapy for residents. • Calculate nutrition requirements and assess adequacy of diet and nutrition support. • Preceptor for dietary managers completing coursework to become certified dietary manager. • Evaluate food handling, sanitation, preparation, service procedures and equipment. • Offer guidance for providing quality and cost-effective food service. • Assist with 4-week cycle menus, adjusting therapeutic and mechanically altered diets as necessary.

• Meets with the Administrator, medical and nursing staff, as well as other related departments in planning food service programs as needed or requested. • Participates, evaluates and implements recommendations from the facility’s Performance Improvement Committee as needed or requested. • Serves on various committees of the facility as required by existing regulations and as appointed by the administrator,. • Assists in developing, implementing, and maintaining an ongoing Quality Improvement Program for the dietary department as needed or requested. • Assures that menus are developed, maintained, approved, and filed in accordance with established policies and procedures and in compliance with physician orders. • Signs and updates Interdisciplinary Care Plans for all residents on a monthly basis. • Provides on-going nutritional consultations for residents as requested by the staff and families. • Obtains and provides documentation of professional liability insurance. • Works in coordination with the Director of Nursing, Administrator and Food Service Manager to assure that facility is in compliance with nutritional-related state regulations. • Reviews tube feeding residents monthly to assure that nutritional needs are met.

• Manage dietary services for Omaha, Plattsmouth, Valley and Nebraska City • Provide services to assigned living centers. • Counsel patients requiring special dietary needs. • Serve as a nutrition resource to departmental and health care staff to enhance nutritional services. • Develop and oversee preparation of specialized diets. • Proficient in long-term care computer data entry (MDS System). • Document approval for all menus, therapeutic modifications, and substitutions used in the facility based off the company Diet Manuel to improve resident meal satisfaction. • Monitor resident eating habits, feeding abilities, and diet compliance to ensure proper documentation meets all reporting requirements.
