* Work with the EBNHC's Let's Get Movin' program (a lifestyle change program) * Outreach to families referred to program by their pediatrician. * Communicate programs offered by Let's Get Movin' (LGM) such as: afterschool programs focused on physical activity and nutrition activities, parent nutrition courses and other community resources. * Help contact families to discuss barriers to program attendance * Use Epic (electronic medical records system) to document initial assesments and other outreach efforts * Use Spanish language skills to communicate with families * Work closely with the Community Initiatives Coordinator for the outreach, enrollment, administration, and fufillment of the Community Supported Agriculture program at LGM. This is one of the many Food Access projects offered by our program. * Assist with the Farmer's Market held in East Boston when needed. * Support the LGM team with program testing; go to afterschool and summer sites to assist with the anthropometric measurements (height and weight) taken for each participant.
