Through Collaborative practice agreement (CPA), serve ~40 clinics across greater Houston area, working with providers, Allied health professionals and specialists across all clinics and a 5-star Medicare advantage plan. Support providers by reviewing patients’ charts, prescribing, substituting, and adjusting doses of medication to improve patient access, and outcomes while maintaining healthcare costs. Serve as liaison between healthcare providers and the Medicare Advantage Plan, optimizing therapeutic outcomes and enhancing medication adherence.

Delivered comprehensive patient care to members by processing and accurately dispensing 500+ prescription orders daily. Ensured compliance with company legal policies, procedures, and regulations for assigned areas. Met or exceeded expected business results while putting customer 1st and consistently applying omni-merchant mindset and EDLP and EDLC business models to all plans. Analyzed, interpreted reports, implemented, and monitored loss prevention and safety controls. Performed annual comprehensive medication review (CMR) and solved dozens of Drug Utilization Reviews daily. Worked with a Care Team and Clinical Administrative Coordinator (CAC) to support patient and provider engagement for Care Transitions Management, Care Coordination, and Medication Therapy Management. Applied clinical knowledge, therapeutic guidelines, and Medication Therapy Management (MTM) skills to avoid drug therapy problems, optimize therapeutic outcomes, ensure patient safety, and improve cost-effectiveness.
