Oversee the residents and staff in a 120-bed skilled nursing facility Ensure adequate staffing ratios Respond to all incidents, such as cardiac arrests, falls, skin tears, medication errors Review all laboratory results and communicate with the physician Obtain new orders, update families, and implement changes Communicate with the pharmacy to ensure medications are available to meet resident needs Complete thorough health assessments, transfer to a higher level of care when appropriate

Perform comprehensive health assessments on hospice patients to ensure comfort and safety Support patients and families throughout the end-of-life process Collaborate with the interdisciplinary team to ensure all needs are met Communicate with the hospice physician, obtain and implement new orders Consult the pharmacist to discuss the efficacy of medications and review treatment plans Make nursing visits in patient’s homes and address symptom management needs 24/7

Perform comprehensive health assessments on adult cardiac patients Monitor laboratory results and report to the provider as needed Monitor cardiac catheterization patients for complications and address accordingly Collaborate with the interdisciplinary team (social work, case management, physician, etc.) Perform discharge planning and education

Complete health assessments and report changes to RN supervisor Administer medications as ordered by the provider Assist with ADLs, ensure resident safety
