Led projects in the quality improvement department with data analytics teams to deliver pertinent info to key decision makers. Using data from Mission Health Systems and Mission Health Partners, developed Care Process Models(CPMs). The CPMs enlisted interdisciplinary staff and used data to deliver the most favorable outcomes for patient care. Managed teams of 8 to 24 physicians and other specialties in an effort to develop CPMs for diabetes, chronic pain, cardiovascular disease, and hypertension. These models used SQL queries, R, Excel, and visualized through Tableau to drive the process to improve patient and billing outcomes. Gathered data from over 160 primary care facilities to map hot spots of chronic disease and deploy tactics to address education, access, and other underlying problems.
