Collaborated with treatment team including licensed clinical social worker, nurses, and counseling staff to develop treatment plans and complete assessments. Conducted home-visits under the supervision of a Licensed Clinical Social Worker. Provided outreach services to Veteran Administration sites and Senior Living Facilities.

Organized events and assessed residents’ abilities, disabilities and best-suited mode of learning, while utilizing the appropriate techniques and skills necessary in a least-restrictive environment. Assisted residents with leisure skills, emphasizing independence and participation.

Project PROMISE: Providing Residential Opportunities for People Living with Mental Illness In Supportive Environments I implemented comprehensive individual assessments, treatment planning, and therapy services to a socio-culturally diverse adult population with Axis I diagnoses. I provided long-term supportive services for clients to live independently within the community with a combination of training and social rehabilitation services that foster capacity and ability to remain in stable, secure, and normalizing environments while focusing on the individual’s strengths, interests, and abilities. I coordinated within the community to assist with the following services: housing, basic needs, entitlements, family functioning, education, transportation, employment, money management, medical/dental, mental health, housing subsidies, medication management, and shopping. I took an active role in spearheading wellness events and activities for colleagues as a Wellness Champion, while acting as an advocate to encourage and promote all programs to host healthy events.

Implemented comprehensive individual assessments, treatment planning, and linkage services to a socio-culturally diverse adolescent population in a School-Based Youth Services Program (SBYSP). Co-facilitated psycho-educational groups and had a committed role in spearheading a psycho-educational anti-bullying group for victimized students identified through New Jersey’s Harassment, Intimidation and Bullying (HIB) policy. Provided short-term support services from a strengths-based perspective with emphasis on abilities and interests. Provided crisis-intervention, conflict management, violence prevention, and drop-out prevention.

I am an enthusiastic, passionate individual who maintains a full caseload, meeting with clients weekly for individual, group and family therapy. I facilitate group therapy to mothers with substance abuse issues at an intensive outpatient level of care. I coordinate administrative aspects of the program to assure compliance with funding/level of service mandates and assure payment for clinical services rendered under vender agreements. Working from a trauma-focused lens, clients are taught and encouraged to practice emotion-regulation skills, as we participate in yoga, mindfulness, and creative arts. I collect productivity reports, as needed, from Clinical staff, submitting monthly and quarterly productivity reports. I oversee the maintenance of a roster of all vender agreement clients with monthly contacts and follow-up with business office regarding billing processes. I work to assist my manager as needed with facilitation of team meetings, assignment of cases and coordination of additional program functions.
