Working with Health People. I am Senior Coordinator of the Diabetes Unit As a team player with Q-TAC and the NY State Academy of Family Physicians and participating Clinical Centers we are Training others with type 2 Diabetes. Once they completed the training they are certified to become a Diabetes Peer Leaders (Coaches). BRINGING knowledge and empowering individuals with information which some or many have but just don't know exactly how to put it into practice. This program can prevent ulcers & amputation. The A1C will go down but also help not only themselves but family members as well. I myself have Diabetes Type 2. My A1C went from 10.4 to 6.4. As I like to say I have Diabetes but Diabetes doesn't control my life I am in control... National Diabetes Prevention Program (NDPP) Lower Extremities Amputation Prevention (LEAP) Diabetes Self- Management Prevention (DSMP) is a step in the right direction .

WORKING WITHIN THE COMMUNITY WHERE THE MOST HELP IS NEEDED TO DEAL WITH DIABETES.

* Enrollments, Assessment, Service Plans, and follow ups. Continuing to assist with goals until met. CM would assist and arrange any of the following with lots of love and commitment: * Housing, 2010E * Homelessness, * Care Providers * Medication * Substance Disorder * Mental Disorder * and/or any other assistance for Individuals coming back into Society........ LOVE the agency and what it has done for so many....

I began my journey with Health People in 1998. I was hired on November 1st as a book keeper and was very good at it but I would walk around on certain days and knew in my heart I really wanted to work as a Case Manager. Well a few things happen here and there and my dream came true. While Working with Health People, I became CM, I was privileged with providing the HIV community Risk Reduction, Access to Care, conducting support group in different shelter systems and giving them a sense of pride because of the stigma in the community due to the lack of knowledge. After 13yrs, I continued my journey with Fortune Society helping individual transition from incarceration back into society PROVIDING services with assistance in reconnecting with Care Providers, assistance with Housing,Health insurance, shelters and Facilitating group like Street Smart and Healthy Living. I have been blessed to work in a field I found very rewarding. I’ve worked in this field for 17yrs and loved it."]
