Erin Jordan

My passion is working with children/youth and families. I determine service needs, goals and address systemic barriers to provide effective program delivery that is client focused. My capacities included making appropriate referrals to community supports in a timely manner. I also utilized strong advocacy skills, lead the development of support plans and determine multi system-level planning while collaborating with community agencies and stakeholders. I effectively support clients to navigate school, health and social services. I provide various educational resources/supports to work towards a successful outcome. In addition, I have developed, revised and deliver presentations and educational sessions within RCC and community agencies. As part of my role, I organized and prepare resources and sessions for the FASD Youth Group. Furthermore, I successfully contribute and co-facilitate the FASD Caregiver Support Group. My participation with the Human Services and Justice Coordinating Committee (HSJCC) has enhanced my knowledge as FASD relates to the Criminal Justice System. Through research, I developed and deliver a well-organized and engaging presentation to the Committee and will be presenting to CMHA’s. In addition, I collaborate with the FASD Team in preparing funding proposals for grants to support the program, planning and activities.

In my Social Work role as Coordinated Access/Intake Worker, I provided assessment, prevention and intervention. I also determined capacities and needs to make appropriate referrals within RCC and to community supports/services.

My role as Program Coordinator included overseeing supports and services for military families in several capacities including program delivery. I provided counselling, information and referral, crisis intervention, mental health, educational workshops, wellness and reintegration.

As a Social Work Caseworker, I supported those facing homelessness, domestic violence, mental health, and chemical dependency. A vital part of the position included assessing capacities and needs to determine effective support strategies. I connected clients to services and advocated for appropriate supports with various agencies. I assisted clients with Housing, Ontario Works, ODSP as well as Immigration and Settlement.

My role with Christian Horizons as a Support Counsellor included all aspects of a DSW working with clients with various developmental and exceptional needs. As Program Coordinator, I oversaw supports and services for the Family Home Program where I managed several homes throughout Windsor-Essex.
