Integrating health and social care

• To provide strategic leadership and overall management for both children’s and adults mental health and wellbeing within NHS Eastern Cheshire’s CGG’s clinical commissioning team in order to secure and maintain appropriate services for the population. • To ensure effective joint working across stakeholders to influence the quality of outcomes for the population and development of effective care pathways in line with the requirements outlined in NHS Eastern Cheshire CCG’s Mental Health commissioning intentions. • To provide lead responsibility for managing complex areas of service redesign and transformation, activity modelling and mobilisation. • To provide overall leadership and management for allocated programmes of work such as the development of integrated pathways between primary care, mental health secondary care providers, the local authority and third sector providers • To provide a management role to associated Boards as a part of the general arrangements. • To develop and implement robust contact monitoring and key performance systems. • To ensure the maintenance of up-to-date and comprehensive needs assessments • To design and develop service proposals consistent with the CCGs responsibilities to promote safe, high quality services in the context of system reform • To ensure procurement processes ensure the provision of services that provide value for money, quality and safety • To ensure that effective monitoring processes are embedded to enable the CCG to review the effectiveness of commissioned services, and to make changes where necessary. • To co-ordinate work with colleagues in other directorates, including finance, public health, clinical and performance. • To ensure commissioning leadership in the process of service development, achieving credibility and confidence amongst clinicians, providers, young people and families
