* This role included but was not limited to: * Assessments (head to toe, CIWA, CAGE, withdrawal symptoms, BAF, WHODAS, MSE, MAST, Level 2: anxiety, depression, mania, sleep disturbance, etc) * Support for the patient and family * Providing interventions for patients in a crisis or to manage withdrawal symptoms * Administered art therapy * Assisting patient safety (locked doors, keep anything with strings or wires off the unit, etc) * Collaborative care with other nurses and physicians

Worked by assessing, treating and creating care plans for patients ranging from birth to 21 years old. Worked on the north side of floor 20 which is focused on the GI and GU systems. Gave support to patients and families, as well as the staff. Treated patients with pharmacological and nonpharmacological methods. For most, took patients to the play room, for walks, and other areas of the hospital to aid in well being without medications. Play therapy offered to most patients and made this role highly enjoyable. Worked with other members of the staff to aid in the emotional, physical and social for each patient. Performed assessments and vitals every four hours on each patients and planned care accordingly. Displayed leadership by being patient advocate, assisting staff without being asked, and quick decision making.

Serving the public in Roger's Park Chicago by visiting patients in their homes and assisting them with their needs on a individual basis. Aiding the patients with everyday care to emergency care. Duties on each visit may include but are not limited to: assessments, plan of care, coordinating future care or appointments with other health professionals i.e physicians, providing services, vitals, referrals, and documentation of visits and/or what has been done.

Responsibilities included but were not limited to: Administering critical medications, starting IV's, maintaining patient safety, collaborating with all staff members to assemble care plans varying by patient, swift decision making in fast paced situations, implementing interventions based on the plan of care, empowering patients through respect for autonomy and advocacy, interpreting (EKG's, CBC's, ABG's and other lab values), inserting foley's, inserting straight catheters, multi-tasking when necessary, and providing respectful standard or care and confidentiality.

This floor was focused on rehabbing patients and getting them to achieve ADL's and walking. Worked collaboratively with physical therapists, physicians and nurses to provide adequate care and make a plan of care for further treatment. Built trusting relationships with patients and family while providing support and comfort to all. Educated patients on plan of care, treatment, and information asked by patients. Assessed all systems for abnormalities, took vitals signs, used analytics to assess conditions and plans of care, administered medications, educated patients for discharge, took care of three patients and assisted with all others when needed, used prioritization of patient need for assignments and used various medical skills depending on patients. Use of HIPPA guidelines and gave equal and fair care to all patients. At the end of shift give SBAR to staff taking over care.
