Healthcare is changing. As our population ages and chronic illnesses become more complex, patients need more than individual healthcare services; they need coordinated systems of care that help them remain safe, independent, and healthy at home. I am a Registered Nurse, healthcare entrepreneur, and Founder & Administrator of Evergreen Nursing Health, a nurse-led home care and home health organization serving Northern Virginia. My mission is simple: To build nurse-led care systems that reduce hospitalizations, improve quality of life, and help people age safely in the place they most want to be - home. With more than 25 years of nursing experience across acute care, ICU, cardiac care, and home health, I have seen firsthand that many hospitalizations can be prevented through early intervention, proactive monitoring, patient education, and coordinated care. At Evergreen Nursing Health, we bridge the gap between clinical care and daily living support by implementing structured nurse oversight, condition-specific care protocols, caregiver education, and personalized care planning. We support individuals who are: Transitioning home after hospitalization Living with chronic conditions such as dementia, diabetes, heart failure, and Parkinson's disease Experiencing functional decline or increased fall risk Requiring coordinated, skilled, and non-skilled care in the home Our philosophy is different.
