Job duties consist of working with the case managers and doctors to set patients in the hospital up with home health care services, IV antibiotics and order equipment needed for the patients. Communication with home health care agencies, DME’s, insurance billing, and patient’s is important part of the job. Duties also consist of ordering wound vacs and other special equipment that is needed. In addition, another responsibility is to talk with families and patients about their options for hospice services and going over the cost and insurance coverage and then getting the patient set up with hospice.
