As a Community Health Field Experience Director, medical students shadow me in the office to receive outpatient based clinical instruction in the care of patients of all ages and conditions of health.

Teach first year (interns) through third year residents the finer points of history taking, physical examination, interpretation of findings and test data, differential diagnosis, and formulation of treatment plans and follow up, with attendant competencies such as interviewing skills, patient education, etc., as it is done in Family Medicine outpatient practice.

Guide other community health centers, migrant health centers, or federally qualified health centers who are members of the Community Health Center Association of New York State (CHCANYS) through the process of obtaining Diabetes Physician Recognition Program (DPRP) by the National Committee for Quality Assurance (NCQA) and American Diabetes Association (ADA). Physicians who achieve DPRP Recognition show their peers, patients and others in the Diabetes community that they are part of an elite group that is publicly recognized for its skill in providing the highest-level diabetes care.

Give clinical support to Outreach Coordinator and Outreach Department team, provide bilingual comprehensive primary care to migrants in Mobile Medical Unit on-site at migrant camps.

Assess and treat children and adults both preventively and using modern disease management paradigms in a Federally Qualified Health Center, Certified by National Coalition for Quality Assurance for management of diabetes mellitus, participant in national Health Disparities Collaboratives in Cardiovascular Disease, Diabetes Mellitus, and Asthma.

I supported providers (Family Medicine, Pediatrics, OB/GYN, Mental Health, Optometry, Dental), participated in the management team with input concerning clinical affairs, recruited clinical providers, developed quality improvement and clinical policies, directed laboratory, and liaised between administration, clinical departments, local and state health departments, other providers of services, and other Federally Qualified Health Centers to coordinate public health efforts and improve healthcare quality.. During this time, I participated in and was champion for one of two Health Disparities Collaboratives (diabetes and cardiovascular), managed multiples sites, and arranged acquisition of the health center's third site, in Lyndonville, NY, an underserved area which was losing its last medical practice, that of David S Bell, MD. This arrangement came to fruition just after my departure at the end of 2009, after which a fourth site was added in Batavia, NY.

Participated with other panel members, some of whom were directors of other health centers, in research, hospital administration, and private practice, to assist the GRHF to choose an area and give input to what measures to achieve in reducing clinical variation (an Institute of Healthcare Improvement goal) between practicing providers: that of treatment of hypertension in the elderly.

Instructed medical students in the diagnosis and treatment of full range of conditions in all ages of family medicine practice.

Determined best practices for the clinics (e.g. Sexually Transmitted Infections, Tuberculosis) and Home Health Agency. Ensured the proper policies and procedures were in place to meet practice and accreditation standards. Consulted on public health/medical cases. Reported directly to Public Health Director Christopher Szwagiel, Ph.D.

Chaired Electronic Medical Record selection committee.

Treated multifaceted conditions, performed 4-10 nuclear exercise and persantine stress tests on a weekly basis, managed staff of 13 employees, hired and supervised three physicians and a physician assistant, managed accounts including cash flow, profit and loss, and capital expenditures. Introduced Synvisc injections, bone mineral density. Negotiated laboratory agreements with LabCorp and Quest Diagnostics. Negotiated with larger group on merge, headed that group's EHR Implementation Team and taught medical students.

Taught a number of medical students outpatient family medicine, both diagnostics and evaluation/management skills, as well as procedures. Worked under Michele R. Lamoreux, M.D.

Accompanied adolescent campers and counselors on combined expedition including Appalachian Trail backpacking and Delaware River Canoeing. Treated acute injuries such as lacerations, sprains, dehydration, hyperglycemia, and provided consultative advice to management of insulin schedules for over a dozen teens.

Learned principles of family medicine, with building on of skills and practices needed throughout career.

Taught advanced computer science undergraduate students modular programming in PASCAL with emphasis on conceptualization, organization, and subdivision of problems, the use of algorithms and data structures, and the importance of effective communication by wave of internal and external documentation.
