Working within the Brevard Senior Nutrition Program, processed client intake information and updated counts and changes for daily Meals on Wheels deliveries for about 100 delivery routes. Received phone calls for nutrition programs, completing intake and referrals for potential clients, and directing to appropriate departments as necessary. Maintained Private Pay client information and logged payments for meals. Completed other administrative/office tasks as needed.

Working within the Brevard Senior Nutrition Program; conducted screenings, assessments, and client updates for the Meals on Wheels and Seniors at Lunch program. Visited clients in the field and at lunch sites, determining client risk level and nutritional need, as well as referring and informing clients of other community resources to meet their needs.

Works with clients and caregivers within Home and Community Based programs, completing 701B assessments, establishing care plans, and connecting clients to services that are appropriate to their needs. Adhered to standards and rules set by the Florida Department of Elder Affairs when conducting assessments, documenting visits, and in authorizing services for clients.
