As a director of clinical services, I manage a full home health care team of 15 employees. Other duties include approving all care plans, orders and medication reconciliation. Responsible for running team and all staff meetings as well as any inservices/trainings. Also responsible for monitoring in home visits by all disciplines including annual employee reviews.

RN case manager managed a full case load of patients with acute and chronic conditions including diabetes, lung disease, heart disease, joint replacement, wound management and intravenous management. Responsible for interdisciplinary communication within home care team as well as between patients' providers. Managed and updated care plans as appropriate and reported to director with any challenges. Participated in weekly team meetings with home health team to problem solve any difficult cases.

I work on a 15 bed gastrointestinal/medical inpatient unit. I primarily take care of GI bleeds, pre/post operative laproscopic surgeries, and respiratory exacerbations.

Assessment skills, IV management, wound/dressing care, drain care, RN to CNA delegation, patient advocate, and patient safety.
