I am currently living in Boise, Idaho where I work at St. Luke's as a Care Coordinator in the Children's Specialty Center. Utilizing a patient-centered model for this medical home, my role is to be an extension of the subspecialty physicians. As a care team we assess the needs of these medically complex children and I then work to link them to the needed services and supports.
